I have a four year child with consolidation of lung. It suggest tuberculous but child has early hepatitis {SGPT - 57}. I want to start AKT. What are your guidelines?

WITH THIS VALUE YOU CAN START ATT.
HRZ ARE MORE HEPATOTXIC.SM AND EMB ARE NON HEPATOTOXIC.

akt can be started but w-frequent monitoring of liver enzymes,you need not discontinue until enzyme levels are 3 times elevated,if so withdraw them and slowly reintroduce once enzyme levels come to nl...

An increase in 2-3 fold in the liver enzymes is not a deterrent to giving standard first line anti TB drugs. However one must monitor the child when on ATT and if the liver enzymes rise further then the drugs need to be changed.
