A.K,8yrs. has SickleCell Disease`he had one crisis in past with dactilitis and shoulder pain` presently fresh crisis severe lt.arm pain` Ibuprofen 6hrly,added Naproxyn` very little relief` in pain since 13, 4, 09` Xray arm NAD` suggest relief.

Expert Answer :
Rule out osteomyelitis. A bone scan would be useful. Also if it is severe sickling, then blood transfusion, correction of dehydration, acidosis may be required.

blood transfusion is indicated for chronic and severe pain
if NSAIDS cannot releif pain,u can use morphine for acute relief
