perinatal hep B does not manifest so early and then when hep causes serous effusion it should be secondary to hypoalbuminemia and henc etransudative in nature.

Congenital or acquired Hepatitis B does not cause isolated pericardial effusion. Since the pericardial effusion was exudative, bacterial infection should be ruled out.
