A male preterm, 3 days old, 1.3 kg weight, suffers from tachypnea RR 86, frequent blood gases always show mild metabolic acidosis in spite of correction, CBC show HB 10.6, WBCs 24.000, PLT 120.000, PT 18 sec, PC 24 percent, the prothrombin conc remain low in spite Fresh Frozen Plasma and vit K, there is no jaundice, there is history of previous sibling death by the same picture.

Expert Answer :
One possibility is Vit K epoxide reductase or gamma-glutamyl carboxylase deficiency but in a preterm, it is difficult to investigate now. FFP support may be given in case of bleeding and a workup can be done once the patient improves.
