A case of mild pleural effusion on left side complicated by collapse 20 days ago with persistently elevated ESR ,-ve tuberculin test in a child 7 y old. No radiological improvement despite treatment with unasyn-cefotaxime for 18 days ,fever recures after control,no toxic manifestations ,no respiratory distress

Expert Answer :
Why is there a collapse in a mild pleural effusion_? Has a middle lobe syndrome been ruled out. Get an HRCT chest to rule out mediastinal adenopathy and underlying collapse. If presence of necrotic and caseous nodes, rule out TB.
