when do we start a child on leukotriene antagonists, dose and precautions to be taken when we start on them

Expert Answer :
Use of Leukotrienes inhibitors in Children.
• In Children with Virus infection triggered wheeze with history of internisttent asthma.
• In persistent mild asthma as a controller alternate to low dose inhaled steroids.
• Add on therapy to inhaled steroids to reduce the dose of steroids.
• In Children of asthma with Allergic Rhinitis.
• In Children with exercise induced asthma.
Dosage
Dosage is age related to age.
• 6 months to 2 Yrs : 4 mg oral granules daily at night.
• 2 Years to 5 Yrs : 4 mg chewable tablet daily at night.
• 2 Years to 5 yrs : 5 mg chewable tablet daily at night.
• 15 Years and over : 10 mg film coated tablet daily at night.
Precautions:
• No safety concerns have been demonstrated in young children
• Zileuton has been associated with liver toxicity.
not in use in India. Please monitor Liver functions.
• Rarely Churg-Strauss Syndrome {vesculitis}, May be due to reduction of steroids when the dose is decreased with the addition of Montelukast.
