there is hyponatremia in DKA for two reaons ,
1. severe dehydration and loss total body loss of sodium
2. pseudo hyponatremia. ie estimation of sodium shows a false low value depending on higher blood glucose
aply correction to get the actual sodium
for every 100 mg of glucose above normal sodium value goes down by 1.6 meq. eg , if glucose is 500 and sodium value comes as 130. correction is calculated as follws .500-100 { if hundred is taken as normal glucose } kmultiplided by 1.6 ie 6.4 to be added to 130 ie actual valie is 136.4 meq.-liter . ie sodium is normal .
if after correction also it is hyponatremic correction as other cases
dehydration loss due to DKA is by osmotic diuresis , more water than electrolites are lost
