A stomach wash in a tertiary centre is not required in all babies until there has been antenatal history of polyhydramnios { to suggest TOF} or preterm delivery with difficulty in breathing or meconium stained liquor.

All New Borns should not be given stomach wash after birth. if amniotic fluid is thick or thin meconium stained, it is advisable to give stomach wash to avoid meconium gastritis in newborn. Even if amniotic flid is stained with meconium but if you feel that is freshly passed{few minutes before birth} and not swallowed by baby you can omit stomach wash.
By passing nasoastric tube at birth sometimes you can early detect T.O. Fistula but incidance is very less. Inspite passing tube, Watch the child for some dribbling secretions from mouth . If mouth is not absolutely clean of secretions you can try nasogastric tube and give stomach wash
By passing tube you may create iatrogenic problem. Incidence of iatrogenic problems will be more then your detected cases of T.O Fisfula
