I am a general pediatrician working in a small set-up. My name is Aasma Rasul. My question is about management of acute severe exacerbation of asthma. My approach to such patients is usually to give frequent nebulized Salbutamol every 20 minutes for 3 to 4 doses and at the same time administraton of steroid. I then continue Salbutamol nebulizer frequently until the patient shows improvement. In such cases at what stage Magnesium Sulphate should be administered_? Is it preferred over Aminophylline_?

Yes you are right.

Only in 1 percent cases may require step III {aminophylline vs magsulf vs iv terbutaline} if treated at right time.

You may choose any one with which you are habituated with. Mag sulp has got some adv {works in a different pathway i.e. ca channel and may not need ICU always}. But the preference is according to your environment of practice. There is no clear recommendation one above the other a sof now.
