Children with TOF typically are born with 4 abnormalities of the heart:

Obstruction of the right ventricular outflow tract
Ventricular septal defect-VSD
Enlargement of the right ventricle
Over-riding of the ventricular septum by the aorta
Knee-to-chest position is advised in a cyanotic spell to increase peripheral vascular resistance. This reduces the right-to-left shunting at the VSD, resulting in increased pulmonary blood flow.

{1}the muscle pump action causes increased return of blood to the right side of the heart and into the pulmonary arteries. {2}also causes increased systemic pressure,causing back pressure over the left side of the heart and thereby decreases shunting through the VSD.
