A 2 day neonate was referred for vomiting and evaluation found to have malrotation gut.
Pre-operative evaluation showed a grossly prolonged PT and APTT, we gave 3 doses Vitamin K and 4 doses of FFP over 4 days with absolutely no improvement of PT and PTT.
The neonate is having no bleeding spots in the skin or no internal bleeding. What could be the causes of persistent prolonged PT and PTT inspire of many doses of FFP_?

Expert Answer :
3 possible causes:
1} Baby may have associated liver problem - which they do sometimes with malrotation.
2} Lab error - Need to recheck Calibration and results, if persistently wrong - this has been found useful in many cases
3} Time to hit books and look for those rare causes - you need a haematologist`s opinion.
Before we discuss further we need the actual results..........and FULL clinical picture - otherwise we are just discussing THEORY
