A 13 YRS OLD GIRL HAS ANASARCA FOR ONE
MONTH.HER
REPORTS ARE AS UNDER-HB-12Gm percent,URINE Albumin 4plus,24 HRS URINARY PROTEIN 1.3 GM,SER CHOLESTROL-160,SER ALBUMIN-3.5GM,S PROTEIN-6.8,S.CREATININE-1.3,URINE PUS CELLS PRESENT.URINE CULTURE and SENSITIVITY STERILE,BLOOD UREA 40MG percent..HER WEIGHT IS 33 KG..USG ABDOMEN FINDING SUGGESTIVE OF RENAL PARENCHYMAL DISEASE..HER BP IS 110, 70..
AFTER GIVING ING CEFTRIXONE,TAB WYSLONE 20MG TDS,TAB LASILACTONE ONE TAB BD PROTEINUREA STILL PRESISTING SHE HAS ALSO GIVEN TAB RAMIPRIL 2.5 Mg OD ALONG WITH THE OTHER MEDICINE.WHAT SHOULD BE FUTURE COURSE OF TREATMENT WHEN PT IS NOT RESPONDING TO STEROID AND RAMIPRIL AND LASILACTONE

Dear Doctor
If this patient is steroid unresponsive and you have excluded all other secondary causes of NS, you should biopsy the child and decide on the use of intensive therapy.
