2 years old male child presents with recurrent eosinophila despite treatment with 2 courses of 21 days hetrazan and 1 short course of steroids. Transient response was seen. What is the further course of action?

What is the exact eosinphil count.have you dewormed the patient and what about the other symptoms of allergy co-existing and have those problems been treated?

a good peripheral smear exam done for any evidence of malignancy. if negative then bone marrow exam should be done before considering the cause of eosinophilia to be a result of any allergic or parasitic cause. Another cause could be a systemic mastocytosis. If all negative repeated courses of anti-parasitic medications repeated every month for 4-6 months may take care of the eosinophilia.

ige levels should be done ,concomitant diseases like atopic dermatitis,asthmatic bronchitis etc should be ruled out a bone marrow aspiration may give clue.

One needs to evaluate the cause of the eosinohilia. Infestations such as toxocara, allergic phenomenon and even Hyper IgE should be ruled out before one labels as Idiopathic hypereosinophilic syndrome. Read the case report on the same on www.pediatriconcall.com or from the link given below:
