There are no controlled studies in children looking at proper dosing of steroids {oral, IV, etc} for nephrotic syndrome. The ISKDS years ago looked at 2 mg per kg per day of steroids {orally} divided TID for new onset NS and that has become the standard of care.
Many nephrologists, if concerned that the patient will not be a routine NS, will often `pulse` the patient with 5-10 mg per kg per dose for 2-3 days {not to exceed a gm per day} of solumedrol for severe diseases. Examples of this would include renal involvement {e.g. NS} with Wegener`s, Lupus, systemic vasculitis or even steroid resistant NS {e.g. Mednoza protocol}.
IV solumedrol should be reserved for non routine NS and in my opinion has little role in standard childhood NS.
