This is regarding anti-tubercular therapy. We have been taught that Pyridoxine supplementation is not required in children when they get INH. But nelson has mentioned that Pyridoxine may required to be supplemented in HIV infected children,Breast fed infants and children taking inadequate diet. IF WE consider these facts then a significant number of pediatric patient will require pyridoxine. Whereas none of the abovesaid children we have prescribed pyridoxine. Kindly clarify

To reduce the risk of drug-related peripheral neuropathy with isoniazid therapy, pyridoxine may be coadministered in all children six years of age and older. In younger children, if there is no malnutrition and dietary intake is good, then no supplement is needed as incidence of neuropathy is less. Pyridoxine administration should also be considered in patients who have conditions in which neuropathy is common {e.g., malnutrition, HIV infected children and patients on anticonvulsants}
