Should quinnie be the 1st drug or artimisinin in complicated malaria?

Since plasmodium falciparum has been found to be resistant to most of the currently used antimalarials, combination therapy with antimalarials is now recommended. Thus, combination therapy may include the following choices:
1. Artemisinin based combination therapy {ACT}
2. Quinine base combination therapy {QCT}
Since Artemisinin derivates have lesser adverse effects as compared to quinine, ACT is preferred for uncomplicated falciparum malaria. For severe vivax or severe falciparum malaria either Artemisinin based combination therapy or quinine based combination therapy may be used.

quinine should be the first line of treatment in complicated malaria as resistance is not yet common in India, and by increasing use of artimisinin we are spending one potent reserve drug.

except in hypotensive and hypoglycemic patients, quinine should be a good drug of course addition of second antimalarial like doxy helps in better clearance.
