Vancomycin is not recommended in the treatment of bacterial meningitis caused by isolates that are susceptible to other agents {i.e., penicillins and cephalosporins}. Even in patients with meningitis caused by highly penicillin- and cephalosporin-resistant strains, Vancomycin should be combined with a third generation cephalosporin and should not be used as a single agent. Meropenem, which has a broad range of in vitro activity and less seizure proclivity than imipenem, has been studied in both children and adults with bacterial meningitis. In these studies, Meropenem has been shown to have clinical and microbiologic outcomes similar to those of Cefotaxime or Ceftriaxone and can be recommended as an alternative to these agents for treatment of bacterial meningitis as a single agent. Meropenam and Vancomycin have been used together in few centres as they have no drug interaction.

meropenem has a good penetrance ti CNS, and is a broad spectrum antibiotic, now Neumococcus o Sthaphilococcus strands associated with meningitis, may require a vancomycin accord with resistence of them in each community profile.
