In a child with nephrotic syndrome with gross edema and the child has started diureting as the proteinuria has started to reduce, but the child is having borderline hypertension which I presume is secondary to the fluid overload caused by the pouring in of extravascular fluids into intravascular compartment as the oncotic pressure builds up. Now this child is having urine output of almost 3000-4000 cc per day. For her hypertension which is preferable- IV lasix or just nifedipine? I am a bit apprehensive to give IV lasix as the child is already pouring out urine. She is 5 years old, dry weight 24 kilos. Thank you.

neither diuretic nor nifedipine is necessary. HTN will comedown along with the edema in a few days with steroid only.

IV Lasix as you are still expecting fluid overload.

The hypertension may be related to the steroids. You may consider nifedepine for control of hypertension as diuretics may lead to dehydration.
