IN A CASE OF PUO if fever is due to ANY auto immune
disease THEN WHAT COULD BE THE POSSIBLE PRESENTATION AND INVESTIGATION FINDING.I SAW A CASE ONE MONTH BACK WHO WAS HAVING PROLONG FEVER HIS WIDAL WAS POSITIVE O TITRE 1, 360 BUT HE DIDI NOT RESPONDED TO INJ CEFTRIXONE,INJ

ETC.NOW I WAS TOLD HE WAS DIAGOSED AS AUTO IMMUNE DISEASE.AT THAT TIME HE DID NOT HAVE ANT RASHES OR JOINT PAIN ETC.I HAVE NOT SEEN THE REPORT OF THAT PT .I JUST WANTED TO KNOW
THE POSSIBLE CAUSE OF AUTOIMMUNE DISORDER WHICH PRESENT AS PROLONG FEVER AND WIDAL POSITIVE.BY FACIES OF PATIENT IT APPEARS HE WAS GIVEN STEROIDS

Expert Answer :
Systemic onset JIA may just present as fever. Rash or joint involvement may occur later. Also in case of prolonged fever, one must rule out hemophagocytosis and also malignancy. Widal may be non specific marker often elevated due to hypergammaglobulinemia due to autoiimmune disease.

in that case detailed history and examination needed....it may be an HEMOPHAGOCYTIC HISTIOCYTOSIS(HLH)...
