How to manage a patient with nephrotic syndrome with very frequent relapses?

A frequent relapser would be defined as a patient having a relapse of more than 2 times in 6 month or 4 times a year.patient should be reinduced with 60mg-m2 prednisolone until he attains remission,and then continued with 40mg-m2 EOD ,reducing the dose of prednisolone 25 Percent every 2-4 weeks and slowly tapering over 6 months.

first rule out organic causes such as SLE, HBV , drugs as addressing this will cure the condition. in case of idiopathic nephrotic syndrome this patient should undergo renal biopsy and the therapy is according to the histopathological diagnosis. this usually involves cyclosporin alongwith steroids
