4.5 y old boy has severe growth failure, both weigt and HT are below 5th centile , he has chronic intermittenet diarrhea which ends with blood {recurs almost every 1-2 months} , he has severe hypoc. microcytic anemia , no dysmorphic features, no skin stigma, no perianal or rectal local lesions , no neurological deficits ,family history non conclosive ,stool analysis shows ameoba hist. how can we approach the diagnosis_?

Expert Answer :
Treat the amoebiasis. Rule out HIV and other immunodeficiency. Also rule out celiac disease and autoimmune diarrhea.

first check the routine inv. for malabsorption like cbc, rft, lft, stool routine, culture.
if no clue then go for stool ph, osmolality, electrolytes, stool for fat, carbohydrates and protein malabsorption.
then progress for further investigation for celiac, cf etc and further proceed for endoscopy
