Spot Diagnosis

Axillary lymphadenopathy

Question :
Spot diagnosis

What is the differential diagnosis?

Discussion :
This child has axillary lymphadenopathy. One must rule out conditions such as tuberculosis, HIV, infectious mononucleosis and even lymphoma. If all serological and blood investigations are negative, a lymphnode biopsy may be required. As seen in the picture, the child has undergone a lymphnode biopsy (white bandage is seen in left axilla) and on lymphnode biopsy was suggestive of a viral infection. Her subsequent CMV IgM was positive. One must never ignore generalised significant lymphadenopathy as it may be a malignant process.
