While giving Inj Penidure,Pencom{benzathine Penicillin} I,M for prevention of recurrence of Rheumatic fever ,the needle often gets blocked and the whole of solutin can not be injected due to its high density. Even recommended amount of distilled water and using 22 Gauge needle fails to solve the problem. What is your experience and suggestion about this practical problem?

by using a wide bore needle,one can avoid blockage,moreover the solution should be shaken well after dissolving and drawn instantaneously.

21 G needle may solve the problem.

shake well for at least 5mts.take two needles one for loading the syringe and another for injecting and inject immediately and as fast as possible.

i think itis better to increased the recomended amount to double the normal and then dividing the dose in two amount egual and each divided dose giving IM in each gluteal muscle...
if this is difficult itis better to give patient aral pencilline or erythromycine preparations as prophylactic instead of benzathine Penicillin
