What is the nursing management of Diptheria and what is the goal and hygiene in the Diptheria?

Expert Answer :
Specific antitoxin is the mainstay of therapy and should be administered on the basis of clinical diagnosis because it neutralizes free toxin only. Efficacy diminishes with elapsing time after the onset of mucocutaneous symptoms.
Pharyngeal or laryngeal (Less than 48 h): 20,000-40,000 U IV infused over 30-60 min
Nasopharyngeal: 40,000-60,000 U IV infused over 30-60 min
Extensive illness (More than 3 d) or brawny neck swelling: 80,000-120,000 U IV infused over 30-60 min
Antimicrobial therapy is indicated to halt toxin production, treat localized infection, and prevent transmission of the organism to patient contacts. C diphtheria is usually susceptible to various agents in vitro, including penicillin, erythromycin, clindamycin, rifampin, and tetracycline.
Patients with difficulty in breathing will require tracheostomy. A watch should be kept for neurological and cardiological complications. Difficulty swallowing and nasal speech are often the first indications of neurologic impairment. Care of tracheostomy tube would be required.
Diphtheria patients need bed rest with intensive nursing care. Patients with laryngeal diphtheria are kept in a croup tent or high-humidity environment they may also need throat suctioning or emergency surgery if their airway is blocked.

care of bedridden pt,like side change,contact points care especially if peripheral neuropathy.

Improve thermoregulation. Maintain room temperature,advise the client to wear thin clothes that absorb sweat easily,encourage to increase oral fluid intake, and administer antipyretics as ordered. Improve caloric intake.

Must PPE to prevent spread of infection, observe vitals especially SPO2 and conscious level, try to control temp.
