Sir,

A 10 yr old male pt . on Tab .Chloroquine andT. Primaquine

as he his reports showed M.P plus, {Pl. vivax},developed

Sub.conjunctival hemorrhage both eyes.

Now, G.C.stable,no other bleeding manifestations,Temp.normal.No h, o trauma.

Now shall we stop PRIMAQUINE, Or continue the

same treatment_? How to proceed_? Is it drug effect_?

Pl clarify_?

Subconjunctival hemorrhage in both eyes may occur due to severe cough as in pertusis or it may rarely a manifestation of bleeding disorder. However it may simulate severe non-purulent conjunctivitis that may be accompaniment of many viral infections. As this child does not have any of the above mentioned risk factors, one should get some more detalis about its origin, duration and progress. Anyway as this child is reported to be otherwise normal both clinically and by laboratory tests, ideal action would be just to observe and not to use any drugs at this juncture nor order any tests. There is no need to interuppt treatment and you may complete the course of drug therapy. Cause of subconjunctival hemorrhage in this child is not clear but one may not consider any action in a hurry and mere observation may help to decide further action. It may be good to know further progress in this child.

we should first check out g6pd status.if normmal go on with same treatment.if g6pd not normal then go for weekly regimen of primaquine therapy

could this be a case of severe , complicated malaria.one should also exclude viral hemorrhagic fever as well.
primaquine may cause intravascular hemolysis in G6PD deficicient individuals

I THINK these drugs are not known to cause such a side effect so other common causes should be ruled out
