i am having 11 year male with lower limb purpuric rashes and bilateral knee and ankle swelling.cbc showing mild thrombocytosis,ASO negative,CRP positive in 1 in 8 dilutions,urine was normal,rft was normal.patient having no fever,just having cold.
my plan is to treat like HENOCH SCHONLEN PURPURA.
my plan is to maintain hydration,paracetamol for pain,antihistaminic for mild itching
kindly advice is there any role of low dose aspirin{81 mg} or any role of oral steroids.
if yes please let me know the duration for which i should use

Expert Answer :
NSAIDS can be used to manage the joint pains. Ge a urine test done to rule out proteinuria and hematuria. Also serum IgA levels to confirm HSP. If there is renal involvement, then steroids are indicated.

there is no role of aspirin.you can use oral steroid which releives abdominal pain fastly and chances of renal involvement is less.

No role for aspirin but steroid may be beneficial to decrease the vasculitis associated with HSP.
