A case of enteric fever with hypothermia . What is the cause and management . This hypothermia was after Nimesulide. Is there any link of Nimesulide in enteric fever to causes hypothermia?

Stop Nimesulide.In a Normal indivisual nimesulide can cause hypothermia.Nimesulide induced hepatic necrosis may add to hypothermia.

I have seen one patient with cold peripheries after administration of Paracetamol. I am yet to use Nimesulide that regularly.
I would like to rule vascular compromise before approaching it as plain hypothermia.
Thalamic syndrome is one possibility because of nimesulide idiosyncrasy

Nimesulide is known to cause hypothermia irrespective of the condition. It also causes cardiotoxicity. Thats the reason why this drug is banned in USA and Uk.

This cud be due to shock caused by enteric toxemia or nimesulide or both. Hepatitis is also more pronounced. Management include stopping the drug and pushing iv fluids and vasopressors.
