2 and half yr old boy who gets recurrent vesiculopustular lesions over palms and soles which is itchy and resolves with a course of Cephalexin and antihistamines so far had a provisional diagnosis of contact dermatitis. They reside in a house attached with ricemill. Earlier he used to walk bare footed in to the mill. Now he gets the similar lesions even with foot wear. Please suggest how to go about!

This patient probably has pompholyx which a form of eczema. Bacterial and fungal infections should be ruled out by skin swab and scin scraping. Biopsy will give definitive diagnosis. Treatment is high potency steroid viz. Betamethasone ointment, Tacrolimus ointment, Potassium permangnate wash. etc.

Rule out pompholyx and allergic contact dermatitis. Pompholyx is a hypersensitive reaction seen often in patients who sweat excessively.
