Anti TB treatment

2yr old girl exposed to father with pulmonary tuberculosis. Started on INH prophylaxis at 3months old for 6mths. However developed cough at one yr. Has one cervical lymph node. Chest Xray revealed no positive findings, mantoux 4mm. However started on Rifampicin, INH and Pyrazinamide for 2mths, then Rifampicin and INH for 4mths, with little improvement.

What next_?

to rule out other causes of cough,size and consistency of lymph node is important you can do absolute eosinophil count and ESR if lymph node big enough go for FNAC or biopsy
