What is the place of `Fractional exhaled nitric oxide` measurement in the diagnosis and management of Asthma_? Is the device available in India_?

Conventional measures of asthma severity have combined assessments of symptoms, amounts of beta2-agonist used to treat symptoms, and lung function. However they do not measure bronchial inflammation. FE,NO may be a quick and simple inflammatory marker with which to assess the impact of treatment changes on inflammation and thus to guide asthma therapy. although large long-term outcome trials are necessary to validate its usefulness. Nitric Oxide
levels in patients with asthma are very sensitive to treatment with inhaled corticosteroids and begin to decrease within 6 hours of treatment and plateau within 3 to 4 weeks. Changes in FE,NO may be an early indicator of loss of control and thus for monitoring control on treatment. Exhaled
Nitric Oxide
has also been used as a diagnostic tool in preschool children. The positive and negative predicted values were 78percent and 95percent, respectively. The clinical utility of routinely measuring FE,NO remains unclear, although current studies are encouraging

Fractional eNO Increased in Br Asthma and following treatment it is Decreased. Exhaled Nitric Oxide is measured by Chemiluminesence Analyser and it is present in India

at present the device is not available in india, but it is expected to be the number one diagnostic modality in years to come...it is easier to measure in children More than 5 years unlike spirometry,,, though at present the cost is prohibitive.
