neonate one week old with severe birth asphyxia .he is a product of cesse to moth with antipartum hge.connected to ventilator on simv mode with pressure support .baby is deeply comatosed with severe edema and his weigt increased six handred gram .on fluid intake restricted as he has renal faliure .he is on antibiotic ,baby on rate 20 per minut oxyg 30percent pip 18 peep 6 ps 10 spont resp not present all blood gases shows resp alkalosis .what is cause inspite of total resp rate is 20. and how to manage pco2 not more than 10. ph 7.48 po2 98 hco3 14 base defi 14 oxy satur 97 wt is 2.950 kg

I think if the baby is ONE week old, you are asking for advise TOO late!! The cerebral edema needs to be avoided in these sort of babies with good volume support to keep the blood pressure well. SEVERE edeme suggest that the baby has been given too much fluids - may need to use diuretics to reduce the edema. You probably needed to transfer these sort of babies to be cared by a NEONATOLOGIST or involve a NEONATOLOGIST in future from day 1 - and telephone call, consultation is better than the emails!!
