at what weight should IUGR neonate be put in incubator and started on IV fluids and what is the intial and subsequent amount of milk should be given.

More than the weight - the temperature instability is the indication for incubator care. IV fluids again is indicated if there is Respiratory distress or if the baby had decreased umbilical blood flow {risk factor for NEC} - otherwise most babies will be able to be fed by NGT initially.
Initially you start @ 60 mls, kg, day and gradually can increase to 150 {to 200} mls, kg, day
