As a resident in a tertiary hospital how should I manage various congenital heart diseases referred to us from other hospitals in the emergency room before I refer them to pediatric cardiology for intervention. I wish to know to which lesions, I should administer oxygen and Prostaglandins as emergency?

Oxygen is required in alll congenital heart disease even if cyanotic if they are in failure or in cyanotic spell. Prostaglandins are required in all patients with duct dependent circulation.
Duct-dependent for systemic blood flow {e.g. Hypoplastic Left Heart Syndrome, critical aortic stenosis, interrupted aortic arch}.

Duct-dependent cyanotic lesions {e.g. pulmonary atresia, transposition of the great arteries}
Over oxygentaion should be avoided.

Prostaglandins will only be useful in neonates in duct dependent lesions. Beware of risk of apnoea. PGs are used until definitive surgery is performed and there should be someone available to intubate if becomes apnoeic.

Ductus dependent lesions like TGA, Tricuspid atresia , hypoplastic left heart syndrome
