A 12 year old male child is suffering from RHD with Mitral regurgitation and Mitral Stenosis. Echocardiogram confirms these valvular lesions and suggests severe Pulmonary artery hypertension. What is the line of treatment in this patient as Valvuloplasty may be contraindicated becase of severe PAH?

How much is the degree of Mitral stenosis {MS} and Mitral regurgitation {MR}? If MS is more than MR, then valvuloplasty should be the treatment of choice as the procedure would reduce the Pulmonary hypertension. If MR is more than MS, then valve repair is indicated to prevent worsening. Medical therapy with vasodilators should continue. Pulmonary hypertension is not a contraindication to valvuloplasty though with high risk.
