8yr old malnourished child with history of fever on and off since 8 months,now presented with severe headache ,eye pain and irritability since 3days.hb low.csf analysis showed 50 cells,polymorphs 80 percent.sugar 45 ,protien 65.afb negative,grams negative.CT scan normal.started ATT and steroids antibiotics antiinflammatory drugs and mannitol.patient responded.he was discharged with oral steroids and ATT AND PYRIDOXINE.child returned back after 2 days with severe headache shouting with pain in head.repeat CT scan was normal.what could be the cause of headache again_?

Does the child have raised ICT_? Rule out cryptococcal meningitis and also bacterial meningitis. Get a CSF done and send for cryptoccocal antigen test, india ink, fungal culture and TB PCR.

disease process perse will take time to respond..as there are no features of raised ICP on CT or presence of granulomas,exudates or hydrocephalus manangement will be conservative
