2 month old female baby, home delivered with uneventful immediate neonatal period brought with jitteriness, tremulousness and repeated opisthotonic posturing from 2nd week of life. she is exclusively breastfed and there is no history of GERD and the weight gain is ok. investigations:sugar, Ur, Cr, Electrolytes, LFT, aminoacidogram, urine metabolic screening and CT Brain were normal. EEG bilateral sharp waves seen.
How i should proceed with the case_?

Get an MRI brain done to rule out brain malformations and workup for metabolic disorders.

what is the acid base status _? metabolic disorder like nonketotic hyperglycenemia,respiratory chain disorder,neonatal ald ,
other rare possibilty of infant botulism , as baby is on breast feed_?_? one of the risk factor ,as there is opisthtonus,. in last what is TT immunisation status of mother.
