Paraldehyde

This is a clear colorless liquid having an ethereal odor and an acrid nauseous taste. It is used a hypnotic and as basal anesthetic.
Route :
Ingestion
Fatal Dose :
60 to 100ml
Fatal Period :
few hrs
Mechanism of action :
It is absorbed rapidly from the stomach and small intestine and also from the rectum. Is is metabolized in liver to trichloroethanol which is hypnotic.
Clinical Picture :
• nausea, vomiting, eructations, headache and giddiness
• breath smells of paraldehyde
• drowsiness, slow pulse, shallow breathing, dilated pupils, cyanosis
• Subnormal temperature, coma


Treatment :
• Gastric lavage with sodium bicarbonate
• Activated charcoal
• high colonic irrigation
• Calcium gluconate and dextrose i.v.
• Symptomatic
