Histrelin

Mechanism :

It is an agonist at GnRH receptors. It acts like a strong inhibitor of GnRH secretion by down-regulating the receptors in the pituitary gland. It is a Synthetic nonapeptide analog & agonist of LHRH/GnRH which desensitizes response to endogenous LHRH.


Indication :

  • Central Precocious Puberty

Contraindications :

• Hypersensitivity

• Caution in case of electrolyte abnormalities

• Caution in case of cardiac conduction abnormalities like QT prolongation, congenital long QT syndrome, family history of QT prolongation, ventricular arrhythmias, history of torsades, decreased heart rate

• Caution in case of history of congestive heart failure, recent MI

• Caution in case of presence of/history of cerebrovascular disease, other CNS diseases, CNS tumors, seizures

• Caution in case of psychiatric disorder

• Contraindicated in pregnancy on the basis of animal data which suggests possible teratogenic potential and fetal death, however, human data is unavailable

• There is no data available on whether histrelin is present in breastmilk, its effects on a breastfeeding infant, or its effects on the production of milk. Health-benefit ratio of breast feeding on the infant, and maternal need for taking histrelin should be considered, however the drug is not usually indicated in breastfeeding women


Dosing :

Dosage forms: Implant: 50 mg which delivers 65 micrograms of drug daily.
Administration: to be inserted subcutaneously in the inner aspect of the upper arm. Care should be taken to ensure that when the implant is removed, the entire implant is taken out.
Central Precocious Puberty:
2 years and older: 1 implant (50 mg) to be inserted subcutaneously for 12 months (changed every 12 months). Can consider discontinuing by the age of 11 years in girls, and 12 years in boys

Adverse Effect :

• Common: Hot flashes

• Uncommon: reaction at the implant site, anemia, fatigue, insomnia, headache, liver disorders, weight gain, constipation, testicular atrophy, decreased libido, erectile dysfunction, gynecomastia


Hepatic Dose :

Not defined for pediatric population
