Diagnostic Dilemma

Non responding fever

A 2.5 years old female child has high grade fever for one month with splenomegaly and some axillary lymph nodes. She has non specific rash and transient mild periorbital oedema. There is transient gum bleeding and swelling {thrombocyte is 197000}. Her CBC is normal, ESR 100 in 2nd hrs, CRP- 35, ASO -ve, cultures are free. She received cephalosporin without improvement.

What is the fiagnosis_?

may be typhoid fever

it could be infectious mononucleosis or systemic onset glomerulonephritis and also we need to rule out malignancy e.g lymphoma
