Diagnostic Dilemma

Fever with desquamation

Author:
Question
A one-year-old girl with fever for 15 days and diarrhea, vomiting, and maculopapular erythematous rash over the body 12 days ago. She was admitted to a hospital and treated with IV antibiotics but fever persisted. There was no cough, urinary complaints, or contact with TB. On looking at the child, she had desquamation of the skin over palms and pink lips.

What is the diagnosis?
Expert Opinion :
This child had a fever with gastroenteritis and rash suggestive of a viral infection. However, on looking at the child there is a desquamation of skin over hands and prominent pink lips with the persistence of fever. Thus one would also consider Kawasaki’s disease in this child (fever + red lips + skin rash + age). In this child, echocardiography showed multiple coronary aneurysms. CBC showed thrombocytosis (platelet count = 12, 23,000/cumm) and ESR of 128 mm at end of 1 hour. She was treated with IVIG & Aspirin following which her symptoms subsided.
