We have a child with hepatospleenomegaly {liver span-12 cms,spleen-5 cms} with no identifiable cause. All infective and metabolic investigations are negative. There is no fever. As such the child had only come for parietal wall abscess and this was only incidental finding. Child has bilateral cervical lymphnodes, mild ascites. Child is on antibiotics and abscess drained. Child is asymptommatic. How do I proceed?

child should b investigated 4 lympho reticular malignancy and primary immunodeficiency disease.

doing peripheral blood smear and bone marrow examination to r,o leukemia

look for imune deficiency diseases mostly do nitro blue tetrazolium test for chronic granulomatous disease

Do CBC and CT chest and abdomen, Did you check EBV antibody titer _?

CBC,P.S.comment,F.N.A.C of lymph node,retic count should be the next line of investigation.if any evidence of hemolysis go 4 HB ELECTROPHORESIS.if nothing abnormal then prceed 4 biopsy of lymph node,spleen,liver ,ascitic fluid analysis to exclude malignancy.chest x ray is mandatory to look 4 mediastinal massalong with U.S.G of abdomen to look 4lymph node and other organs.
