I am a generl pediatrcian working in a small set-up. Last week I had a 6 years old child with typical features of Nephrotic syndrome, generalized edema, hypercholesterolemia, Urine dispstick showing 2plus protien and 24 hours urine protien 31mg, m2, hour. BP is normal, no hematuria. He fits into MCNS except his 24 hours urinary protien excretion which Less than 40 mg, m2, day {nephrotic range}. Whats your opinion_? Will you entertain the same diagnosis_? Or any other concerns should be entertained_?

if the Albumin is low would treat as a straightforward nephrotic syndome and the let the child`s clinical response decide the next step

in this case only the 24hr protein doesnt fit into the criteria of nephrotic syndrome.but still can be treated as nephrotic.suggested to do early morning urinr protein creatinine ratio and if its more than 200 its more suggestive of nephrotic.
