Question
In a case of idiopathic epilepsy disorder (>2 unprovoked seizures), who is seizure free on certain anti-epileptic, if now develops "break- through" seizures because of inter current illness, what should we do? Are there any evidence based guidelines?
Answer
Anticonvulsants have to be contained as there is propensity to convulse still present. Once should also send TDM levels of the ACT and if it is below the therapeutic range, the dose should be adjusted accordingly.
