Dear sir ; Many thanks for all what you are doing to help us... I have a daughter with RTA since her birth. She is on Shol's solution therapy up to 6 months ,she is about 2 years old now & her last blood tests revealed that : Acid / Base : PH 7.53 PCO2 25 PO2 25.8 HCO3-act 20.6 HCO3-std 23.9 Electrolyte: Na+ 138.1 K+ 4.14 Ca++ 4.4 Ca++ (7.4) 4.6 Cl- 102 So, I need your recommendations about these results. Please tell me should I keep on solution, or move to another medication? & if so, what will it be? I want to know, when blood Ca ++ levels decreases, it means it is precipitated in kidney? Please I wait your reply. So many advanced thanks...
Answer
You child's acid balance in the body is doing very well. If she is growing well, you need to continue the same medications as her blood gases are normal on the medication. Regarding the low calcium, she may need calcium supplements as once the acid balance in the body normalizes, the calcium is deposited in the bones. As a result the bones become stronger which otherwise in RTA are osteoporotic leading to bony deformities. You need to give her calcium supplement to maintain the body calcium balance. For the correct dosage, consult your pediatrician.
