Question
I need serious help. My son is 6 years old and we are soon to get out 3rd set of tubes in his ears. Here is our problem. He has fluid in his ears all the time and I am wondering if it is creating more problems. Our Pediatrician has no idea what to do. He is clearing his throat all the time and has thick mucus drainage and it itchy. He has been tested for allergies and has a mold problem. I am at my end. I can't get him to stop clearing his throat. This has been going on for over a year and they have never really tested to see if he has some kind of throat infection. I wonder if his fluid in his ears has a low lying infection in it that is not working with a certain antibiotic. He seems to be getting immune to zithromax and augmentation. Do you have any ideas or suggestions? I am desperate!
Answer
Since you have mentioned that he has had chronic ear effusion requiring repeated tube placement, you need to rule out immune disorders such immune deficiency (i.e. an inherited low immunity of the body) and allergic problems. Since he has already had allergy testing done, primary immune deficiencies should be ruled out. You may need to consult an immunologist for the same. Also, since he has repeated underlying infection, a culture of the ear fluid and also from the throat should be done to find the germ causing the infection and a proper antibiotic for that organism should be given. He may also need to be on long term low dose antibiotic to prevent repeated infections for which oral penicillin or cotrimaxozale may be useful. You will need to do this all under the guidance of a doctor.
