Question
My two year old daughter had a bout of diarrhea last night that was incredibly foul smelling. It smelled like an old outhouse, for lack of a better description. She had some vomiting, but it seems to be secondary to the diarrhea as it was pretty solid vomiting, not the fluidly vomit usually associated with the flu. What could be the cause of this incredibly bad smelling, watery, tan-colored diarrhea?
Answer
Diarrhea with vomiting is suggestive of acute gastroenteritis of which a virus would be the commonest cause. Since, she has a foul smelling stool, bacterial infection should be ruled out. A simple routine stool examination would help and is there is evidence of pus cells in the stool associated with fever, a short course of antibiotics would be required. You must ensure adequate fluid intake less she gets dehydrated and foods such as yoghurt or curd would be preferable.
