Question
My wife is 15 weeks pregnant. Her blood test shows Rubella IgG Ab --- Positive ---- 49.4 IU/ml. ----> 10 indicates positive. Please assist to let us know if this indicates any danger. This is her second pregnancy. Her age is 32. Thanks

A positive Rubella IgG titre suggests either a past infection or a recent infection. A positive IgM titre is more suggestive of a recent infection. Was your wife ever immunized for rubella? (Did she ever receive the MMR vaccine?). If she is immunized, it could be the protective titres of the vaccine. Also did she have any fever with rash or joint pains recently to suggest of rubells clinically? Also, what were the titres in the first pregnancy? If they were positive at that time also, it is suggestive of past infection and there is nothing to worry about. It is more important to repeat the titres 4- 6 weeks later to check if the titres are decreasing or show an increasing trend. If the titres show a decreasing or a table trend, then your wife is immune for rubella and it is suggestive of a past infection or immunity due to vaccine. If the titres increase by 4 fold, it is suggestive of a recent infection. You may then need to consult your obstretician and pediatrician to check for infection in the baby and for further management.
