10yrs male boy getting low grade continuous fever since 2months.Appetite normal no weight loss, not anemic, not citric no lymphedenopathi. Investigations revealed ear 60mm 1st hr, antitubercular ig G is increased. igm is normal. Mx Negative. Hb & tc dc within normal range urine-NAD . Widal 1:40 O positive.Anti rheumatoid factor -ve. Treatment is given. Initially amoxycillin was given no response 2.cefixime was given 7 days no response 3. After anti tubercular ig G positive ATT started since 1month,but there is no response kindly advise
Answer
The child is suffering from PUO. Since all the routine dds have been ruled out, leukemia, connective tissue disorder should be kept in mind. Also sub clinical abscesses should also be ruled out. Anti tubercular igg is positive but that would not be confirmative of Tuberculosis. Rather PCR or Tb Ag would be more useful. A bone marrow, imaging to rule out abscesses and test for connective tissue disorder would be useful.
