3 year old boy is holding back his bowel motions and then does it in his pants. What can be the cause of this?

I have a normal healthy 10 yr. old son and recently had him at the doctor's for a physical. The one problem that I am having with him is that when he had a bowel movement he can't seem to make it to the bathroom in time and has an accident in his pants. When I asked his doctor about this, he said he was just trying to hold it too long and that I should make him go to the bathroom at the same time everyday. Well I really don't think that he is holding himself because lately this has been happening on a daily basis. When he gets to the bathroom, he has a small mess in his pants and then doesn't do anything in the toilet. Could it be that he cannot tell he has to make a bowel movement or have an overactive rectum or something? Is there anything that could be checked to see if there isn't something wrong or do you think he is just too lazy to go to the bathroom?
