Question
I examined a neonate 20 days old weight.3.6kg coming from poor family who had plenty of maggots actively oozing from the umbilicus and prepuce of uncircumcised penis. The periumblical area and glance of penis was inflamed. The baby was otherwise active, feeding well and passing urine normally. The area was cleaned and treated with few drops of formaline solution. Within hour maggots became inactive. After cleaning again antibiotic ointment fusidic acid was applied. Systematic antibiotic was also started. I have earlier seen maggots in nose, ear and wounds but maggots in the umbilicus and under prepuce was seen first time by me. How to manage such a case?
Answer
Maggots can be removed by application of permethrin. However in case of newborn it is best avoided and manual removal is the best bet.
